Self Referral
How does the self-referral process work?
Before submitting your referral to Awakn Clinics, you will need to download and read through our client information leaflet which can be found here for the treatment of Depression, Anxiety and Eating Disorders and here for Addiction treatment. We also have an extensive FAQ section which should be able to answer any of your questions.
Once you have read through the client information leaflet, if you believe you meet the medical eligibility criteria, you need to complete the form below.
Your answers will help us direct your referral accordingly.
To ensure your medical eligibility for receiving Ketamine-Assisted Therapy, we will also conduct a more thorough assessment; including a review of your medical notes held by your GP (with your consent) and, if appropriate, a face-to-face medical assessment held at the clinic.
Please see treatment costs below before answering the questionnaire. Currently this treatment is not covered by NHS or insurance providers.
How much does it cost?
Initial assesment - £350
It is vital that all eligibility criteria have been answered correctly and GP records have been submitted prior to the initial assessment. This is to give the best possible chance of the client being accepted on to the treatment program. The payment for the initial assessment is non-refundable. Payment will not be taken until GP records have been reviewed and our medical team see no immediate eligibility issues. We take these steps in advance to try and reduce the chance of not being accepted. However this is not a guarantee and the medical team will make the final decision at the assessment based on what is best for the client from a medical standpoint.
Full Treatment - £6,995
Once accepted into the treatment course, the cost can be paid in 4 instalments.
